A medical clinic on Squarespace needed a HIPAA-aware multi-step intake form with conditional logic, file upload and a CRM hand-off. Native Squarespace forms don't do any of that.
We injected a React micro-app into the Squarespace page that posts to a serverless backend, encrypts uploads and forwards qualified leads to the clinic's CRM. The form has a 71% completion rate — high for a 7-step, document-heavy flow.
